Clopidogrel (Plavix): It’s a prodrug activated by CYP2C19. Omeprazole/esomeprazole inhibit that enzyme and can reduce clopidogrel activation. If you need a PPI with clopidogrel, many clinicians choose pantoprazole.
Dabigatran (Pradaxa): Needs an acidic environment for absorption. PPIs can lower dabigatran levels slightly, often helpful for stomach irritation, but discuss risks/benefits.
Warfarin: No major direct interaction with PPIs, but any med/diet changes should prompt routine INR checks.
For apixaban (Eliquis) and rivaroxaban, the decision is less about direct interactions and more about whether your bleed risk justifies a PPI.
