Ozempic (Semaglutide): Uses, Dosing, Side Effects & How It Works
Ozempic (semaglutide) is a once-weekly GLP-1 receptor agonist for adults with type 2 diabetes. It helps lower A1c, supports weight loss when paired with diet and exercise, and may reduce the risk of major cardiovascular events in adults with T2D and known heart disease.
What Ozempic Is Used For
- Glycemic control in adults with type 2 diabetes, as monotherapy or with other agents
- Weight reduction support as a secondary benefit alongside lifestyle changes
- Cardiovascular risk reduction in adults with T2D and established cardiovascular disease (per prescriber guidance)
For full consumer information, see the official Ozempic Consumer Product Monograph (Canada).
Dosing (General Information)
Dosing is individualized. Follow your prescriber’s schedule exactly:
- Start: 0.25 mg once weekly for 4 weeks (tolerability step; not for glycemic control)
- Then: increase to 0.5 mg once weekly
- May increase: to 1 mg or 2 mg once weekly if additional control is needed
- Administration: Inject subcutaneously into abdomen, thigh, or upper arm, any time of day, with or without food; use the same weekday each week
Missed dose: Take as soon as possible within 5 days of the missed dose. If more than 5 days have passed, skip and resume on your next scheduled day. Do not take extra doses.
Available Strengths & Pens
- Pre-filled, multi-dose pens designed to deliver the prescribed weekly dose (0.25 mg, 0.5 mg, 1 mg, or 2 mg as directed)
Always verify the correct pen and dose. Follow the Instructions for Use provided with your pen for priming, attaching a new needle each injection, and safe disposal.
Common Side Effects
GI effects are the most common, especially during dose increases:
- Nausea, vomiting, diarrhea, constipation, abdominal pain
- Decreased appetite, headache, fatigue
Seek medical help for signs of pancreatitis (severe abdominal pain), gallbladder problems (pain, fever, jaundice), dehydration (dizziness, fainting), or worsening diabetic retinopathy symptoms. Low blood sugar can occur when Ozempic is used with insulin or sulfonylureas—your prescriber may need to lower those doses.
Who Should Not Use Ozempic
- Personal or family history of medullary thyroid carcinoma (MTC) or with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
- Known hypersensitivity to semaglutide or any component of the formulation
Drug, Food & Alcohol Considerations
- Ozempic delays gastric emptying, which may affect absorption of some oral medicines—monitor drugs with a narrow therapeutic index as directed by your clinician
- Use caution with alcohol, which may increase risk of hypoglycemia when used alongside other diabetes medications
- Tell your clinician about all medicines and supplements you take (including insulin, sulfonylureas, warfarin, and others)
Storage & Handling
- Refrigerate unused pens (2–8°C/36–46°F). Do not freeze
- After first use, most pens can be kept at room temperature (below 30°C/86°F) or in a refrigerator—use within 56 days
- Always keep the pen cap on and protect from heat/light
Alternatives & Related Options
- Rybelsus (oral semaglutide): daily oral GLP-1 option
- Wegovy (semaglutide): higher-dose semaglutide indicated for chronic weight management
- Mounjaro (tirzepatide): dual GIP/GLP-1 agent (discuss with your prescriber)
- Other antidiabetic classes: metformin, SGLT2 inhibitors, DPP-4 inhibitors, basal insulin, etc.
